Through a series of dedicated articles, Alternative Libertaire intends to help
remedy this situation. For this first, we will talk about Hormone Substitution
Treatments (HRT). ---- As part of a gender transition, hormone replacement
therapy (HRT) consists of modifying the body's dominant sex hormones in order to
modify certain secondary sex characteristics. Among the most visible effects, a
masculinizing HRT (THS-M) will cause a change in the voice and extensive
hairiness; a feminizing HRT (THS-F) will cause breast growth; both will cause a
redistribution of fat over the long term. Most effects are reversible, some by
discontinuation of treatment and others via medical procedures. To retain all of
its effects, HRT must be taken for life.

20% fewer depressions
Hormone treatments are often the first step in a medical transition, and are
sufficient on their own for part of the trans population. Their effectiveness is
clear: 20% less depression, in a population 75% affected by this disease and of
which one in two has already attempted suicide (compared to 5% and one in
fourteen respectively in the general population)[1].

However, no marketing authorization (AMM) exists in France for the use of HRT in
the context of transitions. Trans people should therefore use treatments designed
for cisgender people. On the THS-M side, only the injections are easily
accessible, suitable in terms of dosage but leaving little choice on the form. On
the HRT-F side, the treatments available are designed for postmenopausal
cisgender women and their dosage is far too low; all have drawbacks, require
anti-androgens with adverse side effects, and some are regularly in short supply.
It is not uncommon for trans women to obtain injectable estrogens outside
official and non-reimbursed circuits. And let's not talk about the progesterone
intended to be taken vaginally.

The transphobic laws that we see flourishing internationally deprive trans people
of their treatment. This is devastating: a forced stop of HRT is extremely
traumatic. Imagine seeing your body slowly changing into a form that is repugnant
to you... And for people who no longer produce hormones naturally following
surgeries, serious health concerns await them.
